During setup, you'll see checkboxes for all available form fields like Business Name, Contact Phone, Website URL, and Additional Details. Simply uncheck any columns you don't want in your spreadsheet. The workflow will only create columns for the fields you select, keeping your data clean and focused.

Can I add this form to pages other than the Order Status page?

Yes, you can embed the wholesale application form on any page of your Shopify store. While the setup instructions show adding it to the Order Status page in Settings > Checkout, you can paste the same embed code into any page template, product page, or dedicated wholesale page through your theme editor.

What happens if someone submits the form but my Google Sheets integration isn't working?
